Makes the best of a needlessly mediocre design.
Kohler one piece toilets do NOT have an accessible "under-side" which would afford nut access to the bolts holding the seat to the porcelain. Instead, the bolts screw into expanding aluminum anchors that are "clamped" into the seat mounting "blind" hole. For this to work, the anchor must fit thru the 3/8" dia. blind hole and then, with the seat in place, the 1/4" x 20 bolts must be lubricated and tightened, forcing the internally threaded end of the anchor up toward the top flange on the underside of the ceramic hole, expanding the slotted central part to clamp the anchor between the upper flange and the newly formed "fingers". The upper flange is flatted on opposite edges allowing "something" to hold the anchor from rotating while the bolt is being is being tightened from above the seat mounts. It turns out that the underside of the seat mounts have projections that fit over the flange (and it's flats) preventing the anchor from rotating as the bolt is tightened and the fingers are formed. Alternatively, the anchor can be clamped in place without the seat by removing the black plastic insert from the seat mount and using it to hold and form the anchor. Then, when the anchor is tight, the bolt can be removed, the seat mount re-affixed to the black plastic insert, the the bolt re-tightened. This issue could easily have been avoided had Kohler designed the toilet with a solid flat underside in the area where the seat bolts pass thru the porcelain. The blind holes are nice, giving the toilet underside a smooth curve, but, in my opinion, it's hardly worth the fuss needed to install a replacement seat.

Terrible, expensive 16th century technology.
Three-stars because when you finally make it work it does the job it is supposed to. HOWEVER, whoever at Kohler designed this system, and whoever approved it and set it free onto the world, and their respective bosses should all be drawn, hung and quartered. Shame on Kohler for this rip-off, substandard design POS.. I had to replace a cracked seat on my toilet, so first of course, I had to remove the old seat. As soon as I broke the tension on the screw the whole nut shattered. It took me nearly two hours of sweat (and some tears of frustration) to pull this damned thing out, twisting and pulling the screw with a set of pliers. Ordered these ridiculously expensive replacements only to find out I need to spend another $25 on a specialty tool kit. You basically have use this kit to "break" the nut inside the whole to make it anchor. WTF Kohler? It is 2018. By now you should have come up with a polymer anchor that won't rust, last forever, only cost a few cents to make, and readily fit the whole.

Prefect fit
Didn’t think I was going to find these anymore. Have a 1970s solid porcelain Kohler toilet that needed them replaced. Perfect fit a little pricey but great. Hope they last another 50 years

Installs like new.
Update 7/21/20 After 4 years both toilet seats hold as well as when were originally installed. The design of these is well thought and has no issues if properly installed. If you have installed by yourself a faucet or replaced a cartridge in the shower, this will be an easy job for you, but do not hesitate to ask for help - this installation may be frustrating if you have limited experience with mechanical repairs. This is not at all like installing a regular toilet seat where all you need is a screwdriver and have access from the bottom of the hole I have two Kohler toilets and both needed replacement anchors. This anchor worked really well. These anchors come by itself or as a kit. I have found that most handymen should have appropriate tools to install it without a kit. All you need is a wrench, a socket and a regular screw with hex head. You do not need to remove old anchors, when changing the seat if they are intact . This was a case with one of my installations. Only one anchor was broken, thus it needed a replacement. The other anchor was OK and I screwed the new seat into it without a problem. To install the anchor into the toilet, you hold the anchor with a wrench and use a socket to screw in the hex head screw. Once it is installed in place you can remove hex head screw and install your seat. One of my installations required removal of the old anchors. I have found this process to be more difficult then installation. There is a video on the web with instructions that I found to be very helpful. You will need a long crowbar, mine is 36 long. Long crowbar helps to better modulate, i.e. control the amount of force you apply. Use rocking motions and do not try force the anchor out in one push. The video hints about possibility of breaking the toilet if you do not do it just right.

Did the trick
The designer at Kohler that decided to make these fasteners the only way to install a seat should be fired, but my toilet is so old that s/he's probably retired now. Home Depot and Lowed carry Kohler everything, but none of the parts to install one of the old seats. You need special Kohler tools beyond this to get it on. I bought this just to have an extra set in case I screwed up. It took an hour but it worked in the end and we love the new seat that has been falling off for years.

Too pricy for something so cheap
While other toilets have multiple options when it comes to installing toilet seats, Kohler has only one for some of its skirted models, and boy do they make you pay for it. This item is way too expensive and of medium to poor quality for the price. While this anchor works, the first one I bought broke on me and I had to re-order another piece. So far, it has cost me over $60 to replace my toilet seat. Keep in mind, that price is what I paid for these anchors alone - the toilet seat was extra. If these were reasonably priced, I would have given it a reasonable review. Bring your prices down Kohler, there is no way you can justify this kind of money for an anchor.
